We are looking for an experienced Nurse Practitioner to join our team in Caldwell county, NC. The estimated salary for this position is around $140,000 annually, depending on experience.

Job Description: As a Nurse Practitioner on our team, you'll be responsible for providing primary care services to patients in their homes. This includes conducting initial comprehensive assessments, developing patient management care plans, and collaborating with other healthcare professionals to ensure seamless care transitions.
Required Skills and Qualifications: To be successful in this role, you'll need to have completion of a Nurse Practitioner accredited program, current NP license, and current state RN license. You should also have knowledge of CPT, ICD-10, and HCPC codes, as well as awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
