Job Summary:

We are seeking a skilled and compassionate Nurse II-Direct Care RN to join our team at San Antonio State Hospital. As a Direct Care RN, you will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and promoting a culture of respect and compassion.

Duties and Responsibilities:

  • ">
  • Evaluate, treat, and document healthcare needs and/or issues, and report needs and/or issues to the appropriate person(s).">
  • Communicate with others, such as direct care staff, physicians/PCP, administration, and other nursing staff to ensure optimal care as part of the interdisciplinary team.">
  • Notify the supervisor and PCP of trends and concerns to ensure that communication is in place to provide consistent, high-quality care for all persons served.">
  • Assist with services provided by LVNs and Certified Medication Aides (CMA), as needed, and make recommendations for improvements.">
  • Participate in facility quality assurance/quality improvement (QA/QI) initiatives, as required.">
  • Provide treatment to HHS employees injured in course and scope of employment.">

Requirements:

  • ">
  • Licensed to practice as a registered nurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).">
  • At least six (6) months of experience as a practicing registered nurse in a health agency, hospital, or health-care facility.">
  • Bachelor's Degree preferred.">
