Physician Assistant or Nurse Practitioner for Primary Care/Acute Care

About the Role:
We are seeking a highly skilled Physician Assistant or Nurse Practitioner to join our team at Sixteenth Street Community Health Center. As a primary care and acute care medical provider, you will play a vital role in delivering high-quality patient care and contributing to the overall success of our healthcare organization.
Key Responsibilities:- Provide primary care and acute care services to patients with complex health needs, including chronic and acute conditions.
- Collaborate with other healthcare professionals to ensure seamless patient care and continuum.
- Develop and implement effective treatment plans tailored to individual patient needs.
- Evaluate patient data to determine management and treatment options.
- Prescribe medications, recommend therapies, and order diagnostic tests as necessary.
- Active licensure as a Physician Assistant or Nurse Practitioner in the state of practice.
- Certification in Family Practice or Primary Care from a recognized professional organization.
- DEA registration and Basic Life Support certification required.
- Master's degree in Physician Assistant Studies or Nursing Degree in Family Practice/Primary Care.
- Typically 2+ years of experience in primary care and acute care settings.
- Excellent communication, interpersonal, and organizational skills.
Estimated Salary Range: $80,000 - $110,000 per year
