Primary Care and Acute Care Advanced Practice Provider

We are seeking a skilled Primary Care and Acute Care Advanced Practice Provider to join our team at the Sixteenth Street Community Health Center. This role is responsible for providing high-quality patient care in an acute setting, working collaboratively with other medical providers to ensure seamless transitions between care settings.
Responsibilities- Provide limited care coordination and acute care to patients with acute and chronic health conditions in collaboration with other medical providers.
- Evaluate, diagnose, treat, and manage acute patients within an acute care/walk-in setting.
- Act as the primary patient contact, ensuring continuity of care through smooth transitions between care settings.
- Prescribe, recommend, and regulate pharmacologic and non-pharmacologic therapies.
- Order, perform, and interpret procedures and diagnostic tests to manage simple conditions and assist in managing more complex illness and injury.
- Counsel, instruct, and educate patients on physical and mental health.
- Interpret and evaluate patient data to determine management and treatment of patients.
- Physician Assistant or Nurse Practitioner license issued by the state in which the team member practices.
- Family Nurse Practitioner certification issued by the American Academy of Nurse Practitioner Certification Board (AANPCB) or Physician Assistant (PA-C) issued by the National Commission on Certification of Physician Assistants (NCCPA).
- Drug Enforcement Administration (DEA) registration issued by the Drug Enforcement Administration (DEA).
- Basic Life Support (BLS) issued by the American Heart Association (AHA) needs to be obtained within 6 months unless the department leader has determined it is not required.
- Master's Degree in Physician Assistant Studies (may consider Bachelor's degree with 7+ years relevant experience) or Master's/Doctorate Nursing Degree in the specialty of Family Practice/Primary Care.
- Typically requires 2 years of experience in providing and coordinating care for patients with acute and chronic health conditions as a physician assistant or nurse practitioner.
- Demonstrated ability to work independently and as an effective member of a healthcare team.
- Ability to work in a fast-paced, high-demand environment while managing multiple tasks.
- Excellent oral and written communication skills.
- Ability to effectively collaborate with and establish/build relationships with others.
- Proven organizational skills and ability to prioritize effectively.
$115,000 - $140,000 per year, depending on experience and qualifications.
