Key Responsibilities
  • Provide comprehensive care to residential patients, including medication monitoring, education, and prescriber collaboration.
  • Provide information, education, and support for clients, promoting wellness, recovery, and skill building to sustain better health.
  • Monitor service recipient progress in treatment and participate in treatment plan reviews as assigned.
  • Update treatment plans as appropriate and submit treatment plans at all stages for medical review, if applicable.
  • Conduct intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned.
  • Assist service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services.
  • Assist service recipient in making discharge plans and provide follow-up services as assigned.
  • Manage crisis situations, including danger to self and others, to ensure the safety and stabilization of consumers and make appropriate referrals/linkages when clinically indicated.
Requirements
  • Knowledge of guidelines for confidentiality.
  • Knowledge of applicable regulatory guidelines and codes, agency policies and procedures, and Federal and state laws pertaining to position - including SUPR rules.
  • Knowledge of main effects, side effects, and interaction effects of a broad range of psychotropic medications.
  • Knowledge of both psychiatric and physiological causative bases for illness.
  • Skill in written and oral communication.
  • Skill in customer service techniques.
  • Skill in establishing and maintaining interpersonal relationships with clients and staff.
  • Skill in problem-solving.
  • Skill in identifying and prioritizing job tasks.
  • Skill in applying non-violent crisis intervention techniques and maintaining personal safety.
  • Skill in basic nursing techniques.
