About the Role

We are seeking a skilled and compassionate Home Health Physical Therapy Assistant to join our team at LHC Group in Peabody, Massachusetts.

Key Responsibilities:

  • Deliver high-quality physical therapy services to patients in their homes, focusing on improving their well-being and enhancing their quality of life.
  • Work closely with a supervising PT to develop and implement treatment plans that address the unique needs of each patient.
  • Provide exercise instruction, patient education, and support to patients and their families, promoting independence and autonomy.
  • Maintain accurate records of patient progress and communicate effectively with the healthcare team to ensure seamless care coordination.

Requirements:

  • Current licensure as a Physical Therapist Assistant (PTA) or Licensed Physical Therapist Assistant (LPTA) in the state of practice.
  • Valid CPR certification.
  • Access to a reliable vehicle and valid driver's license, with insurance coverage.
