Job Overview

SPIN is seeking a Care Coordinator to support adults with co-occurring dual diagnosis in their journey towards recovery and wellness.

About the Role

The Care Coordinator will work closely with our team to implement quality person-centered Individual Support Plans that are outcome-driven and recovery-oriented. The role requires providing care and support to individuals, maintaining therapeutic boundaries to ensure their safety and meeting their living needs as per the Individual Support Plan (ISP).

Key Responsibilities

  • Implement and monitor each person's ISP to achieve specific outcomes derived from individual preferences, needs, and interests within a trauma-informed recovery-oriented approach.

  • Provide detailed, accurate, and timely documentation for all services and supports provided.

  • Teach and support individuals in life skills to increase their independence and support them to participate in the community, including recreational activities and social events.

  • Support individuals to develop self-directed goals, make informed choices, and follow-through on responsibilities to experience positive self-image, hope, and motivation.

  • Advocate for the individual and/or promote self-advocacy to meet individual needs, facilitate personal growth, and development toward recovery and wellness by assisting in removing barriers or sharing views with team members to create a solution to advocacy concerns.

Requirements

  • A High School Diploma / GED

  • Be at least 18 years old

  • Hold a valid PA/NJ Driver's License with 1 year of driving experience

  • Two years of personal/professional experience working directly with adults with co-occurring diagnoses (dual diagnosis of autism/intellectual disabilities and mental/behavioral health diagnoses)

  • Working experience in crisis intervention, trauma-informed care, and positive behavioral support approaches

  • Covid-19 Vaccination

  • Availability to attend the 2.5 week hourly paid-training program during the hours of 8am-5pm

Salary Range

$16-$20 per hour based on experience, plus additional compensation for weekend shifts.
