Per Diem/On Call RN

As a skilled Per Diem/On Call Registered Nurse, you will play a vital part in our Elderplace team. Your primary responsibility will be to provide high-quality care management and coordination for at-risk frail elder patients with complex medical and psychosocial issues.
We are seeking an experienced nurse who can apply clinical protocols and population-based disease management approaches to improve patient outcomes. You will work closely with our interdisciplinary team (IDT) to ensure seamless care coordination and effective needs assessment.
Key Responsibilities:- Develop and implement comprehensive care plans to meet the unique needs of each patient
- Coordinate access to necessary healthcare services and support
- Collaborate with IDT members to ensure timely and efficient care delivery
- Travel between multiple locations to provide on-site care and support

Benefits and Compensation:We offer a competitive salary range of $40.20 - $63.64 per hour, with opportunities for shift differentials, overtime, and bonuses.
