RN (Per Diem/On Call) - PACE/Elderplace Position

This per diem/on call position is available to support our Beaverton site and surrounding areas as needed.
Critical Care Management is a key component of this role, where the RN provides care management, care coordination, skilled care, and services for at-risk frail elder patients with complex medical and psychosocial issues.
The RN is a core member of the patient-centered PACE interdisciplinary team (IDT) and plays a crucial role in the coordination of health services for patients along the care continuum.
Responsibilities
- Assist providers in identifying needs and coordinating access to appropriate care and services
- Plan, oversee, and monitor all nursing and unlicensed caregiver services for patients on the panel
- Travel between multiple locations as required

Requirements
- Oregon Registered Nurse License upon hire
- National Provider BLS - American Heart Association within 30 days of hire
- Relevant certifications applicable to the position, as specified by the Nurse Manager
