KEY RESPONSIBILITIES
As a Case Coordinator, your duties will include:
  • Collaborating within a team to achieve the office's objectives and workload
  • Establishing and nurturing trusted, client-focused relationships to keep clients engaged in care services as per the Care Program
  • Acting as a resource for technical inquiries, applications, and problem resolution
  • Assessing, Planning, Coordinating, and Managing Homecare Aides to ensure the best interests of both clients and the organization
  • Maintaining a positive relationship with the Community Care Unit
  • Ensuring that client and employee records are thorough and up-to-date
REQUIRED SKILLS:
  • Strong reading and writing abilities
  • Exceptional interpersonal skills
  • Organizational and multitasking capabilities under pressure
  • Basic proficiency in computer and office tasks
  • Ability to delegate effectively
QUALIFICATIONS:
  • Proficient communication skills in English
  • Knowledge of a second language is advantageous
  • High School diploma
