Job Summary

We are seeking a highly skilled and compassionate Care Manager to join our team at Amergis. As a Care Manager, you will play a critical role in coordinating referrals, service planning, and client documentation for designated caseloads.

Key Responsibilities
  • Maintain a full caseload, complete with ongoing individual service plans
  • Assess ongoing changes in behavior, circumstances, or conditions that may affect client safety
  • Engage clients and sponsors in the casework process
  • Prepare, compile, and submit accurate case files
  • Serve as a liaison with stakeholders, including legal providers and immigration court
  • Participate in seminars, workshops, and educational programs
  • Complete home visits to meet with sponsors and clients
  • Maintain contact with clients through phone calls and home visits
  • Consult with supervisors to determine decisions related to client safety and well-being
  • Demonstrate client progress toward risk reduction and achievement of treatment goals
  • Maintain accurate records, files, and statistics in accordance with agency policy and licensing requirements
Requirements
  • Bachelor's Degree in Behavioral Sciences, Human Services, or Social Services fields
  • Experience in Case Management, Behavioral Sciences, Human Services, or Social Services
  • Compliance with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program
  • Current CPR certification (if applicable)
  • TB questionnaire, PPD, or chest x-ray (if applicable)
  • Meet all federal, state, and local requirements
  • Successful completion of new hire training
  • Understanding of patient confidentiality and HIPAA requirements
  • Strong communication skills and proficiency in the English language
  • Computer proficiency
  • Age 18 or older
