Per Diem Hospice Aide / Certified Nursing Assistant

The hospice aide serves as a vital member of the interdisciplinary team, operating under the guidance of a registered nurse to deliver essential services that cater to the personal needs of patients and enhance their comfort. This role involves careful observation of the patient, timely reporting of observations, and meticulous documentation of care provided. The hospice aide is assigned in a manner that ensures the quality, continuity, and safety of patient care.

KEY RESPONSIBILITIES
  • Perform personal care tasks based on patient assignments and respond promptly to their requests. If a task cannot be completed, notify the Case Manager immediately.
  • Ensure the safety of the patient by using equipment correctly and safely (e.g., foot stools, side rails, oxygen).
  • Provide personal care services, including bathing, back rubs, oral hygiene, shampooing, and changing bed linens as required.
  • Assist patients with dressing and undressing as needed.
  • Plan and prepare nutritious meals, including grocery shopping, as assigned.
  • Support patients during meals as required.
  • Take and record vital signs such as oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as directed, demonstrating appropriate competency.
  • Monitor and care for the patient's skin to prevent tissue breakdown over bony areas.
  • Report significant changes in the patient's condition to the Case Manager, while being aware of interpersonal dynamics involving caregivers or others in the patient's environment.
  • Assist with ambulation and exercises as instructed by the hospice nurse or therapist.
  • Provide assistance with bedpans and urinals, and help with light laundry tasks as assigned.
  • Perform range of motion exercises and other simple procedures as part of therapy services as directed, demonstrating the necessary skills.
  • Offer respite for patients, families, and caregivers when present on-site, as appropriate.
  • Maintain cleanliness and orderliness in the patient's living area as assigned.
  • Adhere to the organization's documentation and care standards, as well as professional conduct guidelines.
  • Participate in quality assessment and performance improvement initiatives.
  • Show respect for the patient and their family environment, as well as the personal needs of the patient.
  • Note that the hospice aide will not engage in any activities classified as nursing practices according to the Nurse Practice Act, including administering medications or performing procedures that require the expertise of a licensed nurse.

QUALIFICATIONS (Education, Experience, Knowledge, Skills & Abilities)
  • Completion of a state-approved training program or competency evaluation program.
  • Compliance with state training requirements.
  • Minimum age of 18 years.
  • Ability to read, follow written instructions, and document care provided.
  • Understanding of hospice philosophy and comfort in providing specialized care to terminally ill patients.
  • Positive references from previous employers.
  • Self-directed with the ability to work independently and maintain composure in the presence of death and dying.
  • Empathy for the needs of the ill, injured, frail, and impaired individuals.
  • Current CPR certification.
  • Exhibit tact, patience, and maintain good personal hygiene.
  • Valid driver's license with an insured vehicle that meets organizational requirements.
  • Strong oral and written communication skills.
  • Effective organizational abilities.
