Registered Nurse Pact Case Manager/Care

This is a unique opportunity to join the Department of Veterans Affairs as a Registered Nurse Pact Case Manager/Care. As a key member of our team, you will be responsible for providing high-quality patient care and case management services to our veterans.
Responsibilities- Provide direct patient care and coordinate care with multiple providers across all levels and sites of care.
- Assess psychosocial, nursing, and medical needs of patients and their families/caregivers, and develop individualized care plans to address these needs.
- Collaborate with interdisciplinary teams to ensure timely and appropriate provision of services.
- Evaluate care and outcomes to ensure that services are meeting the needs of our veterans.
- Participate in quality improvement initiatives and contribute to the development of evidence-based practice.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
- One year of specialized nursing experience equivalent to Nurse I, Level 1; OR a Bachelor of Science in Nursing (BSN) with no additional professional nursing experience required.
- A competitive salary and regular salary increases.
