Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a PACT Case Manager/Care Registered Nurse. This is an exciting opportunity to work with a dynamic team and make a meaningful difference in the lives of our patients.

Key Responsibilities
  • Deliver high-quality patient care and coordinate care with multiple providers across all levels and sites of care.
  • Assess patient needs and develop individualized care plans to ensure timely and appropriate provision of services.
  • Collaborate with the interdisciplinary team to provide comprehensive care and address psychosocial, nursing, and medical needs of patients and their families/caregivers.
  • Participate in quality improvement initiatives and contribute to the development of evidence-based practice.
  • Provide education and support to patients and their families/caregivers to promote self-management and customer satisfaction.
Requirements
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • One year of specialized nursing experience equivalent to Nurse I, Level 2; OR a Bachelor of Science in Nursing (BSN) with no additional professional nursing experience required.
Preferred Qualifications
  • Master's degree in nursing (MSN) or a related field.
  • Experience in case management and care coordination.
  • Knowledge of evidence-based practice and quality improvement initiatives.
