Role Overview:

The Senior Care Physician (PCP) serves as a pivotal leader within our collaborative care framework. We emphasize quality over quantity, ensuring our care teams, which include doctors, advanced practice professionals, nurses, and specialists, deliver exceptional service to our patients. This model enables our physicians to manage a reduced patient load, allowing for more meaningful interactions and care.

Key Responsibilities:

  • Assess and manage patients in alignment with established care standards.
  • Monitor patient care quality through data analysis and chart reviews.
  • Facilitate coordination of patient services, including specialty referrals and home health care.
  • Engage actively with the care team during daily meetings to share medical insights.
  • Support the regional medical director in fostering a collaborative practice environment.
  • Ensure timely and accurate medical record documentation, collaborating with coding specialists.
  • Adhere to policies and protocols set by Clinical Leadership.
  • Participate in discussions regarding care quality and outcomes with the regional medical director.
  • Contribute to the development of new services within the Center.
  • Engage in the local on-call program as required.
  • Maintain compliance with all licensing and accreditation standards.
  • Dedicate time primarily to direct patient care, including necessary administrative tasks.

Utilize Your Expertise to Make a Difference

Required Qualifications:

  • Valid and unrestricted medical license; willingness to obtain licenses in other states as needed.
  • Graduate from an accredited MD or DO program.
  • Strong verbal and written communication abilities.
  • Proven interpersonal skills with colleagues and patients.
  • Commitment to an integrated team-based care approach.
  • Adaptability to a value-based care environment.
  • Exceptional patient service skills.
  • Basic proficiency in computer usage, including email and electronic medical records.
  • This position involves patient interaction and requires participation in TB screening.

Preferred Qualifications:

  • Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active DEA license.
  • Experience with Medicare Advantage patient management.
  • Familiarity with Medicare guidelines and HEDIS quality indicators.
