Key Responsibilities:The Nurse Practitioner (NP) plays a vital role within our clinical operations team, delivering direct patient care across various settings, including medical centers, nursing homes, skilled nursing facilities, and home environments. Key responsibilities include:
- Conducting comprehensive geriatric assessments and medical histories.
- Performing physical examinations, diagnosing conditions, and formulating treatment plans.
- Developing individualized nursing care plans and providing health education.
- Facilitating physician referrals and managing case referrals.
- Ensuring thorough documentation in line with ChenMed's quality standards.
- Participating in clinical rounds and team conferences.
The Nurse Practitioner must exhibit the ability to work autonomously while collaborating effectively with other healthcare professionals. This role requires consultation with management and medical staff to maintain compliance with established guidelines.
Primary Duties:The NP will:
- Independently assess both acute and chronic clinical issues.
- Document physical assessments and patient histories while analyzing trends in patient health.
- Implement patient management plans based on collected data.
- Write and manage admission, transfer, and discharge orders.
- Order and interpret laboratory and diagnostic tests.
- Prescribe medications and treatments as appropriate.
- Refer patients to specialists when necessary.
- Maintain detailed progress notes and summaries.
- Engage in patient care rounds and collaborate with multidisciplinary teams.
Additionally, the NP will recognize critical situations requiring immediate physician intervention and will initiate life-saving measures as needed. Strong communication skills are essential for resolving complex issues and enhancing patient services.
Qualifications:To be successful in this role, candidates should possess:
- Current RN licensure and Nurse Practitioner Certification in the relevant state.
- Basic Cardiac Life Support certification.
- Board certification by AANP or ANCC is preferred.
- A current DEA number for controlled substances may be required.
- A minimum of one year of clinical experience is preferred, although strong new graduates will be considered.
