We are seeking a skilled Nurse Care Coordinator to join our team at Cantex. This is an exciting opportunity for a dedicated and compassionate nurse to make a meaningful difference in the lives of patients and their families.

About the Role

The primary functions of this role include administering nursing care for patients of all ages in their place of residence, coordinating care with the interdisciplinary group, patient, family, and referring agency, and assuming responsibility for coordination of care.

This position requires excellent communication skills, assessment skills, and strong clinical documentation skills. The ideal candidate will have 2 years of experience with a Hospice agency, preferably Home Care Home Base experience.

The estimated salary for this role is $65,000 - $85,000 per year, depending on experience. We offer a highly competitive compensation package, including benefits such as:
