Job Title: Mental Health Case Manager

US COMM FOR REFUGE is seeking a highly skilled Mental Health Case Manager to join our team. As a Mental Health Case Manager, you will work with refugees who have experienced trauma related to combat, providing them with access to medical and mental health resources in the community.

Key Responsibilities:
  • Implement a trauma screening tool to identify refugees in need of mental health services
  • Provide direct case management services to refugees, including home visits, intakes, and assessments
  • Coordinate appointments and services, maintain case files and case notes, and provide ongoing individual and group counseling
  • Track client enrollment, outcomes, and interventions, and provide information in regular reports
  • Work with the Wellness Program and resettlement agency to serve refugees with medical and behavioral health needs
Requirements:
  • A Master's degree in social work, mental health counseling, or equivalent
  • Experience in trauma-informed care and strengths-based case management
  • Ability to work as a team member and independently, with high level of self-motivation
  • Exceptional interpersonal skills, including keen listening ability and relationship building
  • Proficient in Microsoft Office
Training Requirements:
  • Satisfactory completion of USCRI's Orientation and Training
  • Complete additional training as identified by supervisor or Human Resources
Physical Demands:
  • Use of manual dexterity, tactile, visual, and audio acuity
  • Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands
