We are seeking a compassionate and skilled Senior Hospice Nurse Case Manager to join our team at Trinity Hospice of Aiken. As a valued member of our interdisciplinary team, you will provide high-quality end-of-life care to patients and their families, ensuring that their physical, emotional, and spiritual needs are met.

About the Role

As a Senior Hospice Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. This includes conducting initial assessments, developing individualized care plans, and providing ongoing evaluation and revision of these plans as needed. You will also collaborate with the patient's attending physician and other members of the Interdisciplinary Group (IDG) to ensure seamless coordination of care.

Key Responsibilities
  • Conduct thorough initial assessments of patients' physical, psychosocial, and environmental needs
  • Develop and implement individualized care plans that address the unique needs of each patient
  • Provide ongoing evaluation and revision of care plans as needed
  • Collaborate with the IDG to ensure seamless coordination of care
  • Maintain accurate and relevant clinical notes regarding patients' conditions
Requirements

To be successful in this role, you will need:

  • Current RN licensure in the state of practice
  • One year of clinical experience
  • Current CPR Certification
  • A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
