Job Description

As a Hospice RN/ RN Case Manager with Gentiva Health Services, you will be responsible for delivering high-quality patient care services, coordinating with healthcare teams, and empowering patient caregivers with education and support.

Key Responsibilities
  • Deliver personalized patient care services, including assessment, planning, implementation, and evaluation of patient care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care delivery and optimize patient outcomes.
  • Educate patient caregivers on disease management, symptom control, and emotional support strategies to enhance patient well-being.
  • Maintain accurate and timely documentation of patient records and communicate effectively with healthcare teams.
