As a Vocational Nurse II, you will engage in advanced nursing duties.

Your responsibilities will include ensuring the care, treatment, and overall well-being of individuals at the State Hospital who present with intricate mental health, behavioral, and medical conditions.

You will perform both routine and complex nursing tasks aimed at enhancing the health of individuals within your designated areas and shifts.

You will also collect specimens for laboratory analysis and conduct health assessments. Additionally, you will guide psychiatric nursing assistants and other staff in delivering services that cater to individual needs. You will execute technical nursing procedures in compliance with facility and state regulations. Staying informed about and adhering to all agency, State Hospital, and hospital policies and procedures is essential. You will provide direction and assist in the on-the-job training of newly hired LVNs and direct care staff.

In collaboration with Registered Nurses (RNs), you will help implement protocols, procedures, and programs designed to enhance the health and safety of both staff and individuals.

You will operate under the general supervision of an RN, exercising moderate discretion and independent judgment.

You may also be assigned additional duties as needed.

These responsibilities may include participating in disaster response efforts or supporting Continuity of Operations (COOP) activation, which may require adjustments to your shift pattern or location.



Key Responsibilities:

Regular attendance is crucial, and you may be required to work specific shifts or, at times, a rotating schedule, extended shifts, and/or overtime in accordance with agency leave policies.


You will administer medications and treatments via various methods, including oral, rectal, internal, or injection.

Collaboration with other medical personnel, such as RNs, Physicians, Advanced Practice Registered Nurses, and Physician Assistants, will be essential in examining and caring for patients.

You will conduct rounds as necessary and may work in different areas of the hospital as required. Demonstrating proficiency in performing complex technical nursing tasks and procedures in accordance with HHSC, hospital, and state regulations is expected.

Training and instructing new LVN staff on nursing functions within the hospital will be part of your role, including familiarizing them with the individuals and their specific needs and preferences.


You will carry out complex nursing procedures according to facility and state regulations, such as wound care and breathing treatments. You will also collect specimens for laboratory testing and perform appropriate screening tests as requested by lab personnel.

Monitoring individuals' weight, diet, meals, and vital signs will be part of your responsibilities. You will assess individuals' health status following medication administration, treatments, and procedures. Instructing psychiatric nursing assistants on how to monitor individuals post-treatment or procedure will also be required.

Timely and accurate documentation of treatments and observations in individuals' medical records (electronic health record or paper chart) in accordance with hospital protocols is essential.

Recognizing symptoms of illness and accurately reporting them to the RN and/or Primary Care Provider is critical.

Promoting interdisciplinary collaboration and integration is vital.

You will communicate relevant information both orally and in writing to other team members, including the RN, to ensure the treatment team has the best information available for decision-making.


You will serve as a resource and mentor for new LVNs, assisting the RN Supervisor with on-the-job nursing instruction and training for new staff.

Supporting RNs in implementing protocols, procedures, and programs to enhance the overall health and safety of staff and individuals will be part of your duties.

You will assist the Infection Control Nurse by implementing measures to prevent disease transmission, contribute to quality assurance programs, and help in the execution of therapeutic nursing programs.

Collecting data for the RN/Primary Care Provider will also be required.

Providing information and education to individuals in line with their recovery plans when performing procedures and treatments, such as explaining medication names and purposes, is essential.


Ensuring that necessary supplies are available in advance for required procedures or treatments is part of your role. You will take inventory and ensure the unit is adequately stocked, ordering supplies through appropriate channels as needed.

Responding to emergency situations to ensure the safety of individuals and adhering to safety and infection control protocols of the hospital is crucial.

Additional duties may be assigned as necessary.

These responsibilities may include participating in disaster response efforts or supporting Continuity of Operations (COOP) activation, which may require adjustments to your shift pattern or location.



Required Knowledge, Skills, and Abilities:

A solid understanding of nursing care standards, procedures, and techniques, along with the principles and processes involved in delivering appropriate patient care.

Familiarity with emergency procedures and equipment, such as CPR, and the ability to assess and care for residents and/or staff members in emergency situations.


Strong reasoning skills to identify problems and consider appropriate resolutions are necessary.

Proficiency in operating computers and relevant software applications is required.

Skills in patient care and treatment are essential.

The ability to provide care to a diverse range of individuals with complex medical, mental health, and/or behavioral health issues is crucial.


Competence in preparing and maintaining records and utilizing an electronic health record system after training is expected.

Effective communication skills and the ability to instruct others are necessary.

The ability to interact appropriately with individuals with intellectual/developmental disabilities, their family members, and staff members is essential.

Physical strength and flexibility to safely perform Cardiopulmonary Resuscitation (CPR) and TIPS are required.
