**Theoria Medical: A Pioneer in Healthcare Innovation**
We are seeking a skilled Medical Director to join our team at Theoria Medical, a leading provider of medical excellence and technological advancement in the post-acute and primary care sectors.
The successful candidate will serve as a key member of our multispecialty physician services team, working closely with nurse practitioners and physicians assistants to deliver high-quality patient care across skilled nursing facilities nationwide.
About the Role:
As a Medical Director at Theoria Medical, you will be responsible for overseeing the clinical operations of our skilled nursing facilities, ensuring compliance with state and federal regulations, and providing guidance to facility staff on quality improvement initiatives.
You will also serve as the attending physician for residents residing at each facility, collaborating with our virtual care team to manage day-to-day resident care and providing comprehensive primary care services through regular rounds.
Key Responsibilities:
- Serve as medical director for skilled nursing facilities (SNFs), participating in administrative and quality initiatives
- Attend and participate in monthly quality assurance and process improvement meetings
- Collaborate with nurse practitioners and physicians assistants to manage day-to-day resident care
- Provide high-quality, patient-centered primary care services through regular rounds
- Diagnose and treat patients with acute illness and chronic diseases

Requirements:
- M.D. or D.O. degree from an accredited institution
- Licensure to practice medicine in the United States
- Board certification in family medicine or internal medicine
- At least 2 years of experience in geriatric medicine or post-acute care
