Job Summary

We are seeking a highly skilled Senior Rehabilitation Therapist to join our team at CVH Care Home Health-Richmond-CA. As a Senior Rehabilitation Therapist, you will play a vital role in providing exceptional patient care and promoting rehabilitation services.

About the Role

This is a travel position with a duration of 13 weeks. You will work on 5x8 day shifts with 40 scheduled hours per week. The shift timing is from 08:00:00-16:00:00, Monday to Friday.

Responsibilities
  • Provide high-quality rehabilitation services to patients in a home health setting.
  • Develop and implement individualized rehabilitation plans to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure seamless coordination of care.
Requirements

To be successful as a Senior Rehabilitation Therapist, you must have:

  • A degree in Physical Therapy or a related field.
  • Current licensure as a Physical Therapist in the state of California.
  • Experience working in a home health setting or similar environment.
