Care Coordinator for End-of-Life Patients

At Interim HealthCare of Sacramento Careers, we are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our interdisciplinary team as a Care Coordinator for end-of-life patients.

About the Role:

We provide home-based care to patients with a prognosis of six months or less to live, focusing on quality of life. As a member of our team, you will work closely with an interdisciplinary team consisting of a physician, RN, CNA, HHA, social worker, chaplain, and volunteers to carry out the patient's plan of care.

Your Key Responsibilities:

  • Provide nursing services that are planned, delegated, and supervised by the registered nurse
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
  • Take a holistic approach to care that engages the patient's mind, body, spirit, and family

What We Offer:

We offer a competitive salary of $32.50 per hour, a schedule of 6 days on, 6 days off, and 1:1 nurse-to-patient ratios where you can make a meaningful impact. You will also enjoy flexible assignments, autonomy, and a great work-life balance.

Requirements:

To be successful in this role, you should have:

  • A graduate degree from an accredited vocational nursing program and an active LVN license in California
  • At least one year of hospice LVN experience
  • CPR certification
  • A valid California driver's license, auto insurance, and reliable transportation
  • Knowledge of state and federal home health and hospice regulations
