About the Role:

CareOne is seeking a highly skilled Physical Therapy Assistant to join our team. As a Physical Therapy Assistant, you will play a vital role in managing the care of our residents, ensuring quality outcomes, and collaborating with our interdisciplinary team.

Key Responsibilities:

  • Provide and direct resident rehabilitation care, meeting all center, state, federal, and accrediting standards.
  • Evaluate residents' needs and establish effective treatment plans.
  • Manage and coordinate the supervision of treatment plans with all staff and other disciplines.
  • Participate in the screening of residents according to directed schedules and practices.
  • Provide feedback to the designated supervisor on clinical and rehabilitation issues.
  • Participate in center-directed processes and services.
  • Show effective management of treatment and non-treatment related activities.
  • Perform necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.
  • Maintain complete, accurate, and timely billing records.
  • Assist with marketing efforts as directed.
  • Participate in in-service training programs as requested.

Requirements:

  • Bachelor's Degree in Physical Therapy.
  • Current and Active Physical Therapy Assistant license within the state of operations.
  • Prefer experience/knowledge in older adult rehabilitation.
  • Prefer knowledge in governmental and regulatory guidelines in designated healthcare settings.
