Primary Care Physician

We are seeking a highly skilled and compassionate Primary Care Physician to join our team at Northwest Human Services, Inc. As a Primary Care Physician, you will be responsible for providing high-quality medical care to our patients, including examinations, diagnosis, and treatment planning.
Key Responsibilities- Provide professional medical services, including examinations, diagnosis, and treatment planning for medical problems.
- Oversee assigned patient care coordination, including specialty referrals and follow-up, case management, and enabling services.
- Consult with nurse practitioners and physician assistants as needed.
- Maintain accurate and up-to-date medical records documentation.
- Contribute to the care team by providing coverage to absent providers and supporting staff.
- Participate in organizational initiatives, such as 340b usage, Patient-Centered Medical Home Certification, and Meaningful Use requirements.
- Graduate of an accredited medical school.
- Completed Primary Care residency (Family Practice, Internal Medicine, or Pediatrics) and board certified.
- Licensed to practice in the State of Oregon with prescriptive authority and DEA.
- Professionally bondable/insurable.
- Ability and willingness to provide obstetrical care preferred.
- Strong interpersonal and communication skills.
- Valid Oregon Driver's License and proof of automobile insurance.
- CPR certification in infant, child, and adult required.
- Bilingual/bicultural strongly preferred.
- Previous experience in community health preferred.
- Ability to function independently and work flexible hours.
