Key Responsibilities:
- Expertly manage both acute and chronic health conditions, ranging from straightforward to complex cases.
- Independently conduct thorough age-appropriate histories and physical examinations for patients with complex acute, critical, and chronic illnesses.
- Order and interpret diagnostic and therapeutic tests tailored to the patient's specific age-related needs.
- Prescribe suitable pharmacologic and non-pharmacologic treatment options.
- Implement strategies to assist patients in regaining or maintaining physiological stability, including serving in a first assisting capacity when necessary.
- Support surgical care delivery in compliance with relevant regulations.
- Evaluate the effectiveness of interventions and adjust care plans accordingly.
- Facilitate smooth transitions for patients within and between healthcare environments.
- Collaborate with a multidisciplinary team, making appropriate referrals as needed.
- Engage in consultations with physicians and other healthcare team members when necessary.
- Provide educational resources to support decision-making for staff, patients, and families.
- Adhere to the professional standards set by governing and licensing boards.
- Demonstrate cultural competence while working with diverse populations.
- Exhibit exceptional interpersonal and communication skills with all stakeholders.
- Balance fiscal responsibility with high-quality patient care.
- Showcase effective time management skills and follow-through on tasks.
- Participate in meetings, mandatory training, and educational programs to remain informed and current in the field.
- Maintain the required continuing education credits as stipulated by relevant governing bodies.
- Stay knowledgeable about and supportive of policies, procedures, and guidelines relevant to the practice setting.
Experience Requirements:
A minimum of 2 years of experience in primary care is preferred.
