We're seeking a highly skilled and compassionate Clinical Assessment Advisor to join our team. As a member of our clinical team, you'll have the opportunity to make a meaningful difference in patients' lives and be part of a dynamic organization that prioritizes whole-person health.

In this role, you'll work closely with patients, families, and healthcare providers to develop and implement individualized care plans that address physical, emotional, and social needs.

Key responsibilities include:

  • Assessing patients' medical conditions and developing comprehensive care plans
  • Leading multidisciplinary teams to ensure seamless care transitions and coordination
  • Collaborating with physicians and other healthcare professionals to ensure optimal patient outcomes
  • Maintaining accurate records and documenting patient progress

Requirements:

  • Bachelor's degree in Nursing (BSN) preferred
  • At least 2 years of experience in clinical home care, with a minimum of 1 year in a leadership or management role
  • Current CPR certification
  • Excellent communication and interpersonal skills
  • Knowledge of state and federal regulations governing home health agencies

Compensation:

The estimated salary for this position is $85,200 - $117,000 per year, depending on experience and qualifications. This is a full-time opportunity with a competitive benefits package, including medical, dental, and vision insurance, 401(k) retirement savings plan, paid time off, and more.
