This role serves as the primary liaison for individuals seeking support with urgent and emergency situations. The position involves welcoming clients, managing phone inquiries, and delivering direct assistance to families and individuals by aiding in application processes, document collection, eligibility pre-screening, and data entry for households requesting services from the Community Assistance Network.
Key Responsibilities
· Provide comprehensive case management services to families, utilizing a family-centered methodology, and support clients in achieving objectives that foster stability, permanent housing, employment, and self-sufficiency.
· Offer guidance, direction, and resource linkage to help clients access community resources and navigate barriers to housing solutions.
· Assist families in completing the universal application required by the organization.
· Aid families in gathering necessary documentation to assess eligibility for support.
· Make referrals to other agencies as needed.
· Input data and scan documents into various management information systems relevant to the services requested by the individual.
· Prepare and submit monthly, quarterly, and annual outreach reports to the Outreach Director.
· Engage in outreach initiatives, including distributing informational materials and speaking to groups about the organization’s programs and services.
· Serve as a positive role model by upholding the highest standards of professionalism and ethical conduct in all interactions with clients and colleagues.
· Maintain regular attendance in accordance with the organization's policies.
· Assess financial assistance needs for clients based on the designated budget for each service area.
· Participate in training sessions, staff meetings, and other professional development activities.
· Collaborate effectively with all staff, supervisors, community professionals, clients, vendors, and the public.
· Adhere to organizational policies and procedures.
· Undertake additional duties and special projects as assigned.
· Conduct thorough needs assessments and create detailed Individual Service Plans (ISPs) for each client, outlining specific goals and action plans aimed at achieving permanent housing and self-sufficiency, focusing on:
Increasing income
Life skills development
Employment and education
Physical and mental health
Financial management
Childcare and parenting support services.
Additional Responsibilities
· Work collaboratively with agency volunteers to enhance their experience while fulfilling program needs.
· May be assigned to outreach offices as needed.
· May provide front desk coverage as required.
· This position may necessitate working evenings, weekends, and other non-standard hours as needed.

Education and Experience
A bachelor’s degree in a human services field from an accredited institution; OR
An associate degree in a human services field from an accredited institution, accompanied by two years of relevant experience.
· Minimum of two years of data entry experience in a fast-paced environment.
· Possession of a valid driver’s license and access to a vehicle for work-related purposes.
· May be required to use personal vehicle for job-related responsibilities.
· Bilingual candidates are preferred.
