Responsibilities:
  • Conduct thorough assessments and develop personalized care plans for patients with complex medical needs

  • Document patient progress and communicate effectively with the care team

Requirements:
  • Active Registered Nurse (RN) License in Colorado
  • Graduate of an accredited school of nursing
  • At least 1200 hours of experience as an RN
  • Healthcare CPR/BLS Certification
  • Home health care experience preferred
