About the Role

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team in a PRN/Part Time capacity. As a Home Risk Assessment Specialist, you will be responsible for conducting comprehensive health assessments in the comfort of patients' homes.

Key Responsibilities:

  • Conduct thorough medical histories, diagnoses, and treatments
  • Provide health education and collaborate with primary care physicians
  • Work in a variety of settings, including home visits, skilled facilities, and telehealth

Requirements:

  • Master's degree in Nursing or equivalent experience
  • Current RN and NP licensure with prescription authority
  • Board certification in a Matrix-approved specialty
  • 1+ year of experience as a Nurse Practitioner
