Diabetes Educator Opportunity at Mercy Primary Care

Mercy Primary Care is seeking a full-time Diabetes Educator to provide comprehensive education and support to patients diagnosed with diabetes in our outpatient setting.

Key Responsibilities:

  • Collaborate with healthcare teams to develop and implement personalized diabetes management plans
  • Empower patients to achieve optimal health outcomes and improve their quality of life
  • Provide education and support to patients and their families

Requirements:

  • Bachelor's degree in Dietetics, Health Education, or related field
  • Certified Diabetes Care and Education Specialist (CDCES) credential required
  • Clinical experience in diabetes management and education, preferably in a hospital or outpatient setting
