Position Summary
The Vocational Nurse Specialist plays a crucial role in delivering direct patient care under the guidance of a Registered Nurse.
Key Responsibilities
The responsibilities include implementing the care plan, administering treatments, and collaborating effectively with the healthcare team to address the needs of patients.
- Formulate a comprehensive Care Plan and ensure interventions for assigned patients are executed in collaboration with the Interdisciplinary Team, patients, families, and attending physicians.
- Maintain precise and timely documentation aligned with the established care plan.
- Conduct thorough nursing evaluations and assessments.
- Oversee the coordination of care for both patients and their families.
- Assist the scheduling team to guarantee the accuracy of the patient's care timetable.
Emergency Response
Manage urgent patient situations, including hospital admissions and necessary behavioral interventions.
- Coordinate services with hospital social workers and other essential community resources, including CBAS, IHSS, MSSP, and waiver programs.
Technical Skills
Utilize computer skills to document all services within the electronic healthcare system/EMR.
- Collaborate closely with medical professionals to advocate for the health needs of patients, families, or caregivers while fostering strong relationships.
Transition of Care
Facilitate effective transitions of care from inpatient or skilled nursing facilities to home or other care levels.
- Supervise field staff to ensure that patient needs are adequately addressed.
- Travel to client residences or facilities as required.
- Accurately document and track activities to ensure proper billing.
- Participate in the coordination of home health services.
- Utilize equipment and supplies efficiently and effectively.
- Provide field supervision to LVNs and Home Health Aides assigned.
- Perform additional duties as required.
