Job Overview

This role involves working directly with at-risk populations, carrying out health promotion programs, conducting basic data collection and analysis, and participating in field work including home visits and other community outreach activities.

Responsibilities
  • Assists families with needs and healthcare coordination regarding the CMH Diagnostic program, CMH Treatment program, and CMH Service Coordination program.
  • Conducts home visits with families and children from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds.
  • Evaluates and performs physical nursing assessments with pediatric clients that have various medical conditions.
  • Interacts with community agencies and the public on a regular basis via telephone communication, email, home visits, and community events.
Requirements

To be successful in this role, you will need:

  • A degree in Nursing from an accredited College of Nursing, with an Associate's degree required for PHN I and Bachelor's degree preferred for PHN II.
  • Current and unrestricted Ohio Registered Nurse license; Ohio Driver's License; BLS CPR Certification.
  • OGBYN, postpartum, or related experience is preferred.
