Job Title: Preservation Case Manager

Samaritas is seeking a skilled Preservation Case Manager to join our team. As a key member of our Family Preservation Team, you will work with at-risk families to provide intensive, short-term or long-term in-home crisis intervention and family education.

Job Summary:

The Preservation Case Manager will provide a range of interventions to children, adults, and families, using acceptable treatment methodologies consistent with the Family Preservation best practice model. This includes providing in-home crisis-oriented treatment, education, and support to family units where one or more family members may be placed outside the home in foster, group, or institutional care or, be at-risk of being removed from the home.

Responsibilities:
  1. Provide in-home crisis-oriented treatment, education, and support to family units.
  2. Actively participate as a member of team meetings; keep all members informed of case status and progress.
  3. Assist clients to clearly define presenting problems, devise methods and goals for resolution, assess their progress toward treatment goals, and plan termination.
  4. Present cases to supervisor for review of treatment plans and progress on a regularly scheduled basis.
  5. Provide appropriate referrals for clients requiring additional or alternative services.
  6. Collaborate with referral sources, agencies, institutions, and individuals who have important roles in the family's life situation.
  7. Complete client follow-up at intervals as outlined.
  8. Complete and maintain all reports, statistics, and assignments as required.
Requirements:
  • Requires a minimum of a Bachelor's degree in any human service field or education.
  • OR Two years' experience working with children and/or families and an associate degree or credit equivalent in human services or education.
