Job Summary

Horn Memorial Hospital is seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing individualized services to patients and their families, addressing their social, emotional, and economic needs in relation to their plan of care and discharge plan.

Key Responsibilities
  • Provide social services to patients and families in the hospital setting, primarily in Med/Surg and Emergency Department, but also responding as needed to other areas.
  • Collaborate with physicians, nurses, and other healthcare personnel to discuss patient plans of care, therapeutic modifications, discharge planning, and needs.
  • Identify patient/family goals and involve them in planning for discharge.
  • Provide basic intervention and counseling as required.
  • Collaborate with Case Manager RN for utilization review, acting as a liaison with insurance companies concerning patient reviews to coordinate services and payment as needed.
  • Identify and communicate recommendations for appropriate utilization of resources to medical staff, administration, and nursing staff.
  • Promote interdisciplinary communication and documentation of the patient's plan of care.
  • Review discharge plan for all patients on a daily basis.
  • Help coordinate transfers to other healthcare facilities.
  • Provide social services in Hospice after/hospital social service needs are met.
Qualifications
  • Education: Bachelor's degree in Sociology, Psychology, or a similar social science field. Master's degree preferred. Social worker or provisional license by the State Board of Examiners for Social Workers is needed, and the ability to obtain/maintain a valid state driver's license.
Experience
  • Minimum of one year case work experience in a healthcare facility, a social service agency, or fieldwork. Able to obtain, verify, and evaluate general and statistical information from a variety of sources. Requires written and verbal communication, organizational, and human relation skills. Ability to use computers.
