Job Summary

We are seeking a highly skilled Occupational Therapist to join our interdisciplinary team at WelbeHealth Careers. As a key member of our clinical team, you will play a vital role in providing quality patient-centered care to our participants.

Key Responsibilities
  • Conduct comprehensive occupational therapy assessments to identify participant care needs, preferences, and goals.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Deliver and document occupational therapy interventions, including the use of devices, techniques, and activities to promote muscle re-education, perceptual-motor and cognitive function, balance, endurance, and other factors affecting independence in performing activities of daily living (ADLs).
  • Supervise therapy assistants, providing training, instruction, and guidance needed to implement functional maintenance programs (FMPs).
  • Provide training and instruction to staff, participants, and caregivers/families in areas of occupational therapy.
  • Collaborate with the interdisciplinary team to support smooth care transitions between settings and participate in end-of-life care coordination and support.
Requirements
  • Bachelor's Degree in Occupational Therapy from an accredited program meeting AOTA & ACOTE standards.
  • Two (2) years of occupational therapy experience in a clinical setting with a frail or elderly population.
  • Two (2) years of supervisory experience with demonstrated ability to mentor and develop team members (preferred).
  • Registration from the National Board for Certification in Occupational Therapy.
  • Licensed and/or eligible for licensure as required in the state of employment.
