Occupational Therapist Job Description

At Veracity, we are seeking a highly skilled Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in coordinating and providing restorative and rehabilitative occupational therapy services to our participants.

Working closely with our Primary Care Physician, rehabilitation team, and other IDT members, you will work to maximize participant independence and safety, and develop or restore function. Your expertise will be essential in developing and implementing individualized care plans that cater to the unique needs and goals of each participant.

Key Responsibilities:
  • Conduct thorough Occupational Therapy assessments to identify participant care needs, preferences, and goals.
  • Develop and implement Occupational Therapy interventions in accordance with physician orders and participant care plans.
  • Supervise Therapy Assistants, providing training, instruction, and guidance as needed.
  • Collaborate with other IDT members to ensure smooth care transitions between settings.
  • Participate in end-of-life care coordination and support.
Requirements:
  • Bachelor's Degree in Occupational Therapy from an accredited program.
  • Two years of occupational therapy experience in a clinical setting with a frail or elderly population.
  • Registration from the National Board for Certification in Occupational Therapy.
  • Licensed and/or eligible for licensure as required in the state of employment.
Preferred Qualifications:
  • Two years of supervisory experience with demonstrated ability to mentor and develop team members.
