Job Summary:The Service Coordinator 2 - Waiver IDD will serve as the primary point of contact for consumers on their caseload, providing guidance and support to ensure they receive the necessary services and resources. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with a diverse range of stakeholders.
Key Responsibilities:- Develop and implement Person Directed Plans (PDPs) that meet the unique needs and goals of each consumer
- Conduct regular assessments and evaluations to ensure consumers are receiving the necessary services and support
- Collaborate with internal and external service providers to ensure seamless delivery of services
- Provide crisis intervention and support as needed
- Maintain accurate and timely documentation of consumer interactions and service delivery
- Bachelor's or advanced degree in psychology, social work, or related field
- 2+ years of experience working with individuals with intellectual or developmental disabilities
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Valid driver's license with a clean driving record
