Job Description:
This role involves working directly with patients to provide compassionate care, educate them on their treatment plans, and coordinate their care with our team of healthcare professionals.
Responsibilities:
- Assist the provider in collecting patient data, including their physical, psychological, social, and cultural dimensions, according to practice standards and policy/procedure.
- Implement age-appropriate interventions based on individual patients' needs as directed by the provider.
- Work closely with the provider to evaluate the effectiveness of care given in progressing patients toward desired outcomes.
- Develop and implement ongoing education and reinforcement of care plans developed by the Provider based on identified health and education needs and the condition and age of the patient.
- Collaborate with the Provider and other healthcare professionals to deliver seamless patient care.
- Assist the Provider with triaging patient portal messages and phone calls.
- Triage phone messages and provide feedback and answers to patient/physician/pharmacy calls.
- Coordinate referrals to specialty services, such as pharmacy, diagnostic facilities, etc. as required by the Provider.
- Perform basic wound care, suture/staple removal and dressing changes as indicated.
- Reconcile medications at point of care.
- Prepare prior prescription authorization requests for Provider review and finalization.
- Manage and monitor providers' clinical inbox in the EHR and triage as necessary per the direction of the provider.
- Assist with preparation of clinical components for Provider review and finalization.
- Preparation of medication refills for Provider review and finalization.
- Assist with blood draws and IV insertions for fluid infusion under the supervision of the Provider.
- Participate in center interdisciplinary team meetings related to patient care plans and outcomes of identified patients.
- Ensure the overall quality of healthcare for each patient.
- Update patient electronic health records with the most recent medical treatments and records from outside facilities.
Requirements:
- Graduated from an accredited Licensed Practical Nurse Program.
- Licensed to practice as a Practical Nurse (LPN) in the States of Alabama and Mississippi (Compact Licensure).
- Minimum Experience: 5 years
- Minimum Certifications and/or Licenses: CPR and first aid certification required.
- Experience with EMR.
- Phlebotomy (blood draw)
- Excellent communication and interpersonal skills.
- Must be organized and attentive to detail.
- Ability to manage competing priorities.
- Resourcefulness in problem solving.
- Able to take and follow through with assigned tasks and accountability.
- Ability to work in a fast-paced environment.
- Experience with Microsoft Office Word, Outlook, and Excel.
- Participates in center and market interdisciplinary teams related to patient care plans, and outcomes.
- Active Listening is essential.
- IV Therapy Certification is required.
- Must be willing to travel between center locations as needed.
Estimated Salary Range:$60,000 - $80,000 annually
