Occupational Therapist

We are seeking a highly skilled Occupational Therapist to work with our interdisciplinary team to provide high-quality care to patients in the Southwest Twin Cities Metro area. As an Occupational Therapist with our team, you will have the opportunity to work independently, visiting patients in their homes and providing tailored therapy services to help them achieve their goals.

• Conduct comprehensive assessments and develop individualized plans of care for patients
• Provide occupational therapy services, including exercises, activities, and education, to help patients achieve their goals
• Collaborate with other healthcare professionals to ensure seamless care coordination
• Document patient progress and communicate with patients, families, and healthcare teams

• Current licensure in the State of Minnesota as an Occupational Therapist (OT)
• Two (2) years of experience as an occupational therapist
• Able to travel within the geographic area serviced by the office
• Valid driver's license, reliable transportation, and proof of auto insurance required
• CPR Certification
