About the Role

We are seeking a skilled Certified Nursing Assistant to join our team in providing high-quality patient care in the outpatient hospice setting.

Key Responsibilities
  • Perform specific patient care procedures under the direction of the primary registered nurse or other licensed clinician.
  • Complete tasks and duties as ordered in conjunction with the physician, in accordance with the patient's care plan.
  • Provide care in the patient's home, which may include single-family residences, Assisted Living Facilities (ALF), or Nursing Homes.
Requirements
  • High School diploma or equivalent.
  • 1 year of direct patient care experience in any clinical setting, or in-home health/hospice.
  • CNA (Certified Nursing Assistant) certification.
  • BLS (Basic Life Support) certification.
  • Microsoft Office skills.
  • Ability to learn and document in EPIC charting system.
  • Good driving record, access to a vehicle, and a valid driver's license.
