Job Summary

We are seeking a dedicated Social Worker to join our team in Annandale, VA. As a Social Worker with Revival Home Health & Hospice, you will be responsible for providing exceptional patient care and developing customized treatment plans that deliver real results.

Job Responsibilities

• Guide and counsel individuals and families in crisis, addressing various challenges such as unemployment, family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments

• Assist clients in obtaining government funds, education, or treatment, evaluating their needs and capacities

• Develop service plans designed to restore patients to optimum social and health adjustment within their capacity

• Refer clients to community resources and other organizations, and liaise with other social service agencies to ensure comprehensive support

• Prepare patient histories, service plans, and reports, reviewing service plans and performing follow-up to determine quantity and quality of service provided to clients and status of client cases

Requirements

• Master's Degree in Social Work

• Five plus years clinical social work experience

• Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license
