We are seeking a compassionate Clinical Social Worker to join our team in Annandale, VA. As a Clinical Social Worker with Revival Home Health & Hospice, you will play a vital role in delivering exceptional patient care and support to individuals and families in need.

Job Description

The ideal candidate will possess a Master's Degree in Social Work and have at least five years of clinical social work experience. A valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license is also required. As a key member of our team, you will be responsible for:

  • Guiding and counseling individuals and families in crisis, addressing personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments
  • Assisting clients in obtaining government funds, education, or treatment
  • Securing information and evaluating factors contributing to client situations
  • Developing service plans to restore patients to optimum social and health adjustment within their capacity
  • Liaising with other social service agencies and community resources to assist patients in resuming life in the community or learning to live within limits of disability
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Master's Degree in Social Work
  • Five plus years of clinical social work experience
  • A valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license
  • The ability to make psycho-social assessments and develop and implement viable care plans
