As a Care Coordinator, you will serve as a trusted guide for patients, advocating for their needs while they face serious health challenges, including severe illnesses and complex medical conditions.
Your clinical knowledge and holistic approach will empower patients with the necessary education and resources to navigate the intricate healthcare landscape.
Core Duties:
- Identify patients requiring targeted case management and chronic condition oversight based on established criteria.
- Utilize nursing methodologies to effectively manage patient care and ensure seamless integration of health services.
- Provide comprehensive education and support to both patients and healthcare providers to facilitate a smooth healthcare experience.
- Uphold the highest standards of care by adhering to therapeutic protocols, evaluating health outcomes against established care objectives, and making necessary adjustments to care plans.
- Document all relevant interactions and activities in appropriate systems to maintain accurate patient records.
- Foster collaborative relationships within the healthcare team through effective communication and problem-solving.
- Assist patients with wellness initiatives and available benefits.
- Perform additional responsibilities as required.
Qualifications:
Licensure:
Must possess an active and valid Registered Nurse (RN) license in Ohio.
Education:
A Bachelor of Science in Nursing (BSN) is preferred.
Certification:
Active Case Management Certification (CCM) is preferred.
Experience:
A minimum of 2 years of clinical experience in direct patient care is required; 5 years is preferred. Experience in case management or program coordination is also preferred.
Skills:
Excellent verbal and written communication abilities are essential. Must be comfortable engaging with patients and providers via phone. Ability to convey complex clinical information in an understandable manner to non-clinical audiences is crucial.
Proficient administrative skills, with a strong command of Microsoft Office Suite (Outlook, Word, Excel, PowerPoint), and familiarity with virtual communication tools (Teams & Zoom) is necessary. Trustworthiness and the ability to handle confidential information responsibly are essential attributes for this role.
