Nurse Care Specialist

We are seeking a skilled Registered Nurse to join our team at Assured Home Health, a part of LHC Group. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes. Your primary focus will be on delivering compassionate and personalized care to patients with diverse needs.
Responsibilities
• Conduct comprehensive assessments of patients to identify their unique care requirements
• Develop and implement personalized care plans in collaboration with physicians and other healthcare professionals
• Provide direct patient care, including administering medications, wound care, and other treatments as prescribed
• Educate patients and their families on self-care techniques and disease management
• Collaborate with the healthcare team to ensure seamless care coordination and communication
Qualifications
• Current RN licensure in the state of practice
• Current CPR certification
• Reliable transportation and a valid driver's license
