Responsibilities- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Perform focused interviews to identify specific patient needs and develop personalized care plans.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition, and collect, analyze, and interpret data to inform care decisions.
- Document actual and/or potential nursing diagnoses, and collaborate with healthcare team members to ensure comprehensive care.
- Perform other nursing duties as assigned, including administering medications, wound care, and vital sign monitoring.
- Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) License in the state in which the LPN practices.
- Current Health Certificate (per facility).
- Current PPD or Chest X-Ray.
- Current BLS card.
