We are seeking a skilled Physical Therapist to join our Home Health team at Hackensack Meridian Health. As a Physical Therapist, you will play a vital role in providing high-quality rehabilitation services to patients in their homes.
Responsibilities- Conduct comprehensive patient evaluations to identify rehabilitation needs and develop individualized treatment plans.
- Implement and progress treatment plans to achieve patient goals and outcomes.
- Collaborate with multidisciplinary teams to ensure effective care coordination and resource referral.
- Provide education and training to patients and caregivers on proper techniques and strategies for optimal recovery.
- Document patient progress and outcomes in accordance with organizational policies and procedures.
- Bachelor's Degree in Physical Therapy (PT) or Doctorate in Physical Therapy.
- Minimum of six months of experience as a Physical Therapist.
- Current Physical Therapist License and AHA Basic Health Care Life Support HCP Certification.
