Job Summary

We are seeking a skilled Physical Therapist to join our Home Care team at Hackensack Meridian Health. As a Physical Therapist, you will provide professional rehabilitation therapy to patients in their homes, evaluating, developing, and implementing individualized goal-oriented therapy programs.

Key Responsibilities
  • Complete patient evaluations within specified time frames, ensuring comprehensive and holistic assessments of clients' conditions.
  • Develop and implement individualized treatment programs based on evaluation findings, focusing on time-oriented, functional, and measurable short- and long-term goals.
  • Prepare and submit accurate and timely documentation, including progress notes, reassessments, and discharge summaries, in accordance with organization policies and procedures.
  • Act as a liaison among physicians, medical care providers, and other healthcare team members to ensure effective coordination, resource referral, and utilization of home health aide services.
  • Supervise and educate patients and caregivers on written home programs, providing comprehensive education based on identified needs.
Requirements
  • Bachelor's Degree in Physical Therapy (PT)
  • Minimum of six months of PT experience
  • Physical Therapist License and AHA Basic Health Care Life Support HCP Certification
