  • Provide support and guidance to participants and their families, helping them to navigate community resources and access necessary services.
  • Collaborate with community partners and other stakeholders to ensure a comprehensive and coordinated approach to service delivery.
  • Conduct regular home visits and community outreach to engage with participants and their families, building trust and fostering positive relationships.
  • Maintain accurate and timely records of participant progress and service delivery, ensuring compliance with program requirements and regulations.

Requirements:

  • High school diploma or equivalent required; bachelor's degree in a related field preferred.
  • Minimum 1 year of experience in a related field, such as social work, counseling, or youth development.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and mutual support.
  • Valid driver's license and reliable transportation required.
